In the laboratory of pathology in AZ Delta, there is research on tissues and body fluids. The laboratory can be divided into four sections. The first part of the laboratory is the room where the tissue is cut into smaller pieces. These smaller pieces are then places in cassettes. The next part of the laboratory is the room where the cassettes are embedded. After the tissues are embedded, they can be cut in sections. All sections are stained with the hematoxylin-eosin staining or other additional staining’s. The third part is cytology where the body fluids are processed. And finally, there is immunohistochemistry where the immunohistochemically staining’s are executed.
The purpose of this bachelor test is to make a risk analysis about tests that take place on external samples. Many errors can occur during this process that can have major consequences for the patient. This is why a risk analysis is drawn up to find all possible errors and to find appropriate actions so that these errors can be avoided in the future.
To make the risk analysis, the Failure Mode and Effect Analysis (FMEA) method was used. This method describes all possible risks with the aim of reducing these risks. The first step is describing the different process steps in chronological order. Once all process steps have been described, there can be looked what can go wrong during each individual process step. For every possible error, there can be looked at the probability that this error occurs, the discovery and the consequence of this error. A Risk Priority Number (RPN) value can be calculated based on the chance of occurrence, the discovery and the consequence. If the RPN value is too high, an action must be taken with the aim of reducing these risks.
Many errors can occur during tests who are performed on external samples. Most errors were observed when filling in the application form and during processing the sample. The biggest consequences are sample or patient change which will make the RPN high.
